Request for Three-Phase Electric Service
Select the appropriate form based on your request. Your options are new three-phase electric service or upgraded three-phase electric service.
Both forms are in Adobe Acrobat format. You have two options for completing the form:
- Print out a copy, complete all of the requested information and fax or mail the form to PPL Electric Utilities. Fax number and mailing address are shown on the form.
- Type the requested information directly onto the form using Adobe Acrobat by tabbing from one category to the next. Print out the completed form and fax or mail it to PPL Electric Utilities. Fax number and mailing address are shown on the form. Please note that you will not be able to save your completed form electronically.
